Referral Form

Please include a basic patient history, imaging, laboratory results and previous medical records where possible.

Referrals for visiting specialists should be booked directly with the relevant specialist service. If you are unsure who to contact, please get in touch and our team will be happy to point you in the right direction.

For Emergency & Critical Care (ECC) referrals, please call our hospital and speak directly with one of our veterinarians before submitting your referral. Patient histories, diagnostic results and supporting documentation can then be emailed to admin@nqcare.com.au
